In 2001, the Foundation established a Memorial Program commemorating loved ones.
Memorial Gifts will receive recognition in one of five categories:
1. For gifts up to $249.00 the names of the persons memorialized will be added to the Donor Book.
2. For gifts totaling $250.00 to $499.00 the names of the persons memorialized will be displayed on the board for one year.
3.
For gifts totaling $500.00 to $999.99 the names of the persons memorialized will be displayed for three years
4. For gifts totaling $1,000.00 to $4,999.00 the names of the persons memorialized will be displayed for five years.
5. For gifts of $5,000.00 or more and the names of the persons memorialized will be displayed permanently.
Before the names of persons memorialized are removed from the boards, every effort will be made to ensure the next of kin is notified with regard to the policy.
Names will be permanently added to the Donor Book when removed from the Board.
Hand-crafted wooden memorial boards and a beautifully bound donor book are located in the main lobby of Cowichan District Hospital. Here names of loved-ones are displayed by category, as memorial donations accumulate.
